Services

The following services are offered (Case Management Services, Information and Referral Services, and Healthcare Services):

Case Management Services- The staff assisted patients/clients with obtaining resources to maintain their housing/residence, address their mental health and behavioral health needs, purchase medications, obtain medical equipment, transit services, scheduling of medical appointments, obtaining food stamps and other financial supports, obtaining companionship and homemaker services, and completion of comprehensive needs assessment to address the patients/clients psychosocial needs—emotional, cognitive, behavioral, etc.

Information and Referral Services- The staff assists patients/clients and their support system with identifying community resources or supports that can be beneficial to them, and can address their immediate unmet needs. The staff also walks the patients/clients through the processes of obtaining resources—assistance with completing paperwork and/or documents, assistance with follow-up telephone calls, obtaining transportation services to and from appointments, etc.

Healthcare Services- In collaboration with Advanced Clinical Consultants, home healthcare providers and other service providers in the Greater New Orleans area are able to provide home-based medical screening services and healthcare medical services to the patients/clients in a timely manner. Patients/clients are able to remain in their homes and receive all supportive services and primary care.
